Billing and insurance information MUST be provided prior to your appointment day. Due to in-office time constraints, we will be unable to verify benefits on the day of your appointment. If insurance information is not provided at least 24 hours before, please expect to pay out of pocket and self-submit your claim.
Retinal Imagery is required by our offices, and is not covered by most insurances.
If you are more than 10 minutes late for your check-in, we may have to reschedule.
All no-shows and cancellations with less than 24 hours notice are subject to a $50 fee due before you schedule your next appointment.
Out-of-Network Patient Acknowledgement: If you have any of the following insurance plans (VSP or Microsoft EyeMed), we are out of network. By booking, you are agreeing to paying the full price of the exam and understand you will be reimbursed by insurance based on your out of network benefits. Please reach out to your insurance for specific reimbursement amounts.
Please be advised that as of June 2024, our in-network insurance participation has changed. Coverage that was accepted prior to this date may no longer apply.
