After scheduling your appointment, please fill out the Patient Forms with your insurance information.

1. Billing and insurance information MUST be provided prior to your appointment day. Due to in-office time constraints, we will be unable to verify benefits on the day of your appointment. If insurance information is not provided, please expect to pay out of pocket and self-submit your claim.
2. Our office is located inside Costco but we are independently owned, and hence are in network with different insurances than Costco Optical. A listed insurance plan does not guarantee coverage. Please contact the office directly, if you are concerned about coverage and benefits. The only way to know for certain if we are an in-network provider is to send us your insurance information 48hrs prior to your appt for us to verify your benefits. Also note that we are no longer in network with VSP.
3. Retinal Imagery is required by the doctors in our clinics and is not always covered under insurance. Fees range from $25-$40.
4. If you are more than 10 minutes late for your appointment, we may have to reschedule.
5. All no-shows and cancellations with less than 24 hrs notice are subject to a $15 fee due on your next appointment.
